NH Medication Bridge Program
For Providers: Getting Started

Where should you start?
It is very important to make sure that the providers serving your patients are involved in the implementation of the program. The program will not be successful unless they agree to participate in the process. Most applications require a provider signature and most companies send the medications to the provider for him/her to dispense, so it is imperative that they are willing to participate.

It is equally important to get the 'buy in' of everyone that will be involved in the program. This may include the front office staff that will be checking the medication in, the hospital pharmacist who will be counseling the patients, the case managers who may be referring patients to the program, etc. This program will touch all aspects in the continuum of care. It is important to educate all involved on the specifics of the program to ensure smooth implementation and continued success.
